The preface
Health is a necessary requirement for promoting all-round human development and a basic condition for economic and social development. To achieve a long and healthy life for our people is an important symbol of national prosperity and rejuvenation, and is also the common aspiration of the people of all ethnic groups in China.
The Party and the state have always attached great importance to people's health. Since the founding of the People's Republic of China, especially since reform and opening up, remarkable achievements have been made in the field of health reform and development. The environment in urban and rural areas has improved significantly, the national fitness campaign has developed vigorously, the medical and health service system has been improved day by day, and the health level and physical quality of the people have been continuously improved. In 2015, China's average life expectancy reached 76.34 years, and the infant mortality rate, the mortality rate of children under the age of five, and the maternal mortality rate fell to 8.1 per 1,000, 10.7 per 1,000 and 20.1 per 100,000, respectively, which is generally better than the average level of middle - and high-income countries, laying an important foundation for building a moderately prosperous society in all respects. At the same time, industrialization, urbanization, population aging, changes in the spectrum of diseases, the ecological environment and lifestyle have also brought a series of new challenges to maintaining and promoting health. The contradiction between the overall inadequate supply of health services and the growing demand for health services remains acute, and the coordination between health development and economic and social development needs to be improved. Major and long-term issues of relationship health need to be addressed in a coordinated manner at the national strategic level.
To build a healthy China is an important foundation for completing the building of a moderately prosperous society in all respects and basically realizing socialist modernization. It is a national strategy to comprehensively improve the health quality of the Chinese nation and coordinate the health of the people with economic and social development. It is a major measure to actively participate in global health governance and fulfill the international commitments of the 2030 Agenda for Sustainable Development. The next 15 years will be an important period of strategic opportunities for building a healthy China. The upgrading of the consumption structure will create broad space for the development of health services. Scientific and technological innovation will provide strong support for improving health standards. A more mature and established system in all areas will provide a strong guarantee for sustainable development in the health sector.
In order to promote the building of a healthy China and improve the health of the people, the outline of the Plan is formulated in accordance with the strategic deployment of the Fifth Plenary Session of the 18th CPC Central Committee. This outline is a grand blueprint and action program for building a healthy China. The whole society should strengthen its sense of responsibility and mission, make all-out efforts to build a healthy China, and make greater contribution to the great rejuvenation of the Chinese nation and the progress of human civilization.
Part 1 Overall Strategy
Chapter 1 Guiding ideology
We will carry out the "five-sphere integrated plan" in a coordinated manner and the "four-pronged Comprehensive" strategy in a coordinated manner, earnestly implement the decisions and plans of the CPC Central Committee and The State Council, adhere to the people-centered development philosophy, firmly establish and implement the new development philosophy, adhere to the correct guidelines for health and health work, focus on improving people's health, and take institutional reform and innovation as the driving force. With the focus on popularizing healthy life, optimizing health services, improving health security, building a healthy environment, and developing the health industry, we should integrate health into all policies, accelerate the transformation of the development model in the health field, maintain and protect people's health in an all-round and whole-cycle manner, significantly improve health standards, and significantly improve health equity. It will provide a solid and sound foundation for realizing the "Two Centenary Goals" and the Chinese Dream of great national renewal.
It mainly follows the following principles:

We should give strategic priority to health development, integrate the concept of health promotion into the whole process of public policy formulation and implementation based on our national conditions, accelerate the formation of a healthy lifestyle, ecological environment and economic and social development model, and achieve sound coordinated development between health and economic and social development.

We should follow the law of development in the health sector, give priority to prevention, combine prevention and treatment, and place equal emphasis on traditional Chinese and Western medicine, change service models, build an integrated medical and health service system, and transform health services from extensive development featuring scale expansion to green and intensive development featuring improved quality and efficiency. We should promote the complementary and coordinated development of traditional Chinese medicine and Western medicine, and improve health services.

We will promote equal access to basic public health services, with a focus on rural areas and the community level, maintain the commonweal nature of basic medical and health services, gradually narrow the differences in basic health services and health levels between urban and rural areas, regions, and people, achieve universal health coverage, and promote social equity.
Chapter 2 strategic themes
"Joint contribution and shared benefits, health for all" is the strategic theme of building a healthy China. The core is to take people's health as the center, adhere to the grass-roots as the focus, reform and innovation as the driving force, prevention, traditional Chinese and western medicine as the priority, integrate health into all policies, the people build and share the benefits of health work policy, targeting the lifestyle, production and living environment, medical and health services and other factors affecting health. We should combine government leadership with mobilizing the initiative of the society and individuals, encourage everyone to participate, make efforts and enjoy, give priority to prevention, promote healthy lifestyles, reduce the incidence of diseases, and strengthen early diagnosis, treatment and rehabilitation, so as to achieve universal health.
Joint contribution and shared benefits are the basic path to building a healthy China. We should make efforts from both the supply side and the demand side, coordinate the three levels of society, industry and individuals, and form a strong synergy to maintain and promote health. We should encourage broad participation of the whole society, strengthen cross-departmental collaboration, deepen integrated military-civilian development, mobilize the initiative and creativity of social forces, strengthen environmental governance, ensure food and drug safety, prevent and reduce harm, effectively control ecological and social and environmental risk factors affecting health, and form a multi-tiered and diversified social governance pattern. We need to promote supply-side structural reform of health services. Sectors such as health care and family planning and sports should take the initiative to meet people's health needs, deepen institutional reform, optimize the allocation of factors of production and service supply, strengthen weak links in development, and promote the transformation and upgrading of the health industry to meet people's growing health needs. We should strengthen individual responsibility for health, improve the health literacy of the whole people, guide the formation of a healthy lifestyle that is independent, self-disciplined and in line with our own characteristics, effectively control the factors of life and behavior that affect our health, and create a social atmosphere in which we love, pursue and promote health.
Universal health is the fundamental goal of building a healthy China. We will provide equitable and accessible, systematic and continuous health services based on the whole-population and whole-life cycle, and achieve a higher level of health for all. We should continue to improve systems, expand services, and improve the quality of services so that all people can enjoy quality and affordable health services for prevention, treatment, rehabilitation, and health promotion. We should give priority to addressing the health problems of key groups such as women and children, the elderly, the disabled, and low-income groups. It is necessary to cover the whole life cycle, identify a number of priority areas for major health problems and influencing factors at different stages of life, strengthen interventions, and realize whole-course health services and health protection from the fetus to the end of life, so as to comprehensively safeguard people's health.
Chapter 3 Strategic objectives
By 2020, a basic medical and health care system with Chinese characteristics covering both urban and rural residents will be established, health literacy will continue to improve, health services will be improved and efficient, and everyone will have access to basic medical and health services as well as basic sports and fitness services. A health industry system with rich connotation and a reasonable structure will be basically in place, and major health indicators will be among the highest in middle - and high-income countries.
By 2030, the system for promoting health for all will be improved, the development of the health sector will be more coordinated, healthy lifestyles will be popularized, the quality of health services and health protection will continue to improve, the health industry will flourish, health equity will be basically achieved, and major health indicators will join the ranks of high-income countries. By 2050, China will build a healthy country in line with a modern socialist country.
To achieve the following goals by 2030:
-- People's health has continued to improve. People's health has improved significantly, with average life expectancy reaching 79.0 years by 2030, and average healthy life expectancy rising significantly.
- Major health risk factors are effectively under control. People's health literacy has been greatly improved, healthy lifestyles have been popularized, a healthy working and living environment has taken shape, food and drug safety has been effectively guaranteed, and a number of major diseases have been eliminated.
-- The capacity of health services has been greatly improved. A high-quality and efficient integrated medical and health service system and a complete public fitness service system have been established, the health care system has been further improved, the overall strength of health science and technology innovation ranks among the top in the world, and the quality and level of health services have significantly improved.
-- The health industry has expanded significantly. A healthy industrial system with a complete system and an optimized structure has been established, and a number of large enterprises with strong innovation ability and international competitiveness have been formed, becoming a pillar industry of the national economy.

Section 1 Improving the health literacy of the whole people
We will promote the nationwide campaign for a healthy lifestyle, strengthen family and high-risk individuals' guidance and intervention for a healthy lifestyle, and carry out special campaigns for a healthy weight, healthy mouth and healthy bones. By 2030, all counties (cities and districts) will be covered by the campaign. Develop and disseminate appropriate technologies and supplies to promote healthy living. A system for releasing core information on health knowledge and skills has been established, and a nationwide monitoring system for health literacy and lifestyle has been improved. Establish and improve the system of health promotion and education, improve the ability of health education services, and popularize the knowledge of health science from an early age. We will strengthen cultural and ethical progress, develop a healthy culture, change customs and foster good living habits. Media at all levels and of all types have intensified the publicity of health science knowledge, actively built and standardized various radio and television health programs, and expanded health education through new media.
Section 2 Strengthen school health education
Bring health education into the national education system and regard health education as an important content of quality education in all stages of education. We will establish a mechanism for promoting health education in schools, especially in primary and secondary schools. To construct a health education model that combines teaching of related subjects with educational activities, classroom education with extracurricular practice, and regular publicity and centralized publicity and education. To cultivate health education teachers and include health education into pre-service education and post-service training of physical education teachers.
Chapter 5 Shaping healthy behaviors of autonomy and self-discipline
Section 1 Guide reasonable diet
We have formulated and implemented national nutrition plans, carried out in-depth studies on the nutritional functions of food (agricultural products and foodstuffs), popularized knowledge of dietary nutrition, issued dietary guidelines suitable for the characteristics of different populations, guided residents to form scientific eating habits, and promoted the construction of a healthy eating culture. We have established and improved a nutrition monitoring system for residents, and implemented nutrition interventions in key areas and populations, focusing on addressing the problems of micronutrient deficiency and the excessive intake of fat and other high-calorie foods by some people, so as to gradually address the coexistence of insufficient and excessive nutrition among residents. Implement clinical nutrition intervention. We will strengthen guidance on nutrition and health work in schools, kindergartens and elderly care institutions. Demonstration canteens and healthy restaurants will be built. By 2030, people's nutrition knowledge and literacy will be significantly improved, the incidence of diseases related to nutritional deficiency will be significantly reduced, per capita daily salt intake will be reduced by 20 percent, and the growth rate of overweight and obese population will be significantly slowed down.
Section 2 Tobacco control and alcohol Restriction
We will comprehensively promote the implementation of tobacco control agreements, intensify efforts in tobacco control, and improve the effectiveness of tobacco control by means of pricing, taxation and laws. Further publicity and education on tobacco control will be carried out. We will actively promote smoke-free environments and strengthen supervision and law enforcement over tobacco control in public places. We will promote smoking ban in public places, and gradually realize total smoking ban in indoor public places. Leading officials should take the lead in banning smoking in public places and turning Party and government offices into smoke-free institutions. Strengthen smoking cessation services. By 2030, the smoking rate of people over 15 years old will be reduced to 20 percent. Strengthen health education on alcohol restriction, control alcohol overuse, and reduce alcoholism. Strengthen monitoring of harmful use of alcohol.
Section 3 Promoting Mental health
We will strengthen the construction and standardized management of the mental health service system. We will increase public awareness of mental health and improve mental health literacy. We will strengthen intervention in common mental disorders such as depression and anxiety, as well as psychological and behavioral problems, and strengthen early detection and timely intervention of psychological problems in key groups. We will strengthen reporting, registration and management of treatment and assistance for patients with severe mental disorders. We will comprehensively promote community rehabilitation services for mental disorders. Improve the intervention ability and level of psychological crisis in emergencies. By 2030, interventions for the prevention and treatment of common mental disorders and the identification of psychological and behavioral problems will be significantly improved.
Section 4 Reducing Unsafe Sex and drug hazards
To strengthen comprehensive social management, focus on adolescents, women of childbearing age and the floating population, carry out publicity, education and intervention on sexual ethics, sexual health and sexual safety, strengthen comprehensive intervention on those with high-risk sexual transmission behaviors, and reduce unintended pregnancies and the transmission of sexually related diseases. Efforts should be made to disseminate knowledge about the dangers of drugs, countermeasures and ways to treat them. We will strengthen the construction of a national drug treatment medical service system to detect and treat addicts as early as possible. To strengthen the linkage between drug maintenance treatment and community detoxification, compulsory isolation and community rehabilitation. Establish a drug rehabilitation model that integrates physical detoxification, psychological rehabilitation, employment support and social return to minimize the social harm caused by drugs.

Section 1 Improving the national fitness public service system
We will coordinate the construction of public fitness facilities for all, and strengthen the construction of fitness trails, riding paths, fitness centers, sports parks, and community multifunctional sports arenas. By 2030, a network of public sports facilities at the county and village levels will be basically completed, with a per capita sports field area of no less than 2.3 square meters, and all urban communities will be covered by 15-minute fitness circles. We will open public sports facilities free of charge or at a low fee, and ensure that all public sports facilities and those of enterprises and public institutions that meet the opening conditions are open to the public. We will strengthen the building of the national fitness organization network, and support and guide the development of community-level sports organizations.
Section 2. Carry Out the national fitness campaign extensively
We will continue to formulate and implement national fitness programs, popularize scientific fitness knowledge and fitness methods, and make fitness a part of our daily life. Organize social sports instructors to carry out extensive national fitness guidance services. We will implement national standards for physical exercise, develop fitness and leisure activities for the masses, and enrich and improve the national fitness system. It has issued guidelines on physical fitness activities, established and improved exercise prescription libraries for different groups, different environments and different physical conditions, promoted the formation of a disease management and health service model that combines physical and medical services, and brought into play the positive role of scientific fitness for all in health promotion, prevention and rehabilitation of chronic diseases. We will strengthen the development of scientific and technological innovation platforms for national fitness and scientific fitness guidance service sites. China has carried out national physical fitness tests, improved the physical health monitoring system, developed and applied big data for national physical health monitoring, and carried out risk assessment of sports.
Section 4 Promoting sports activities of key groups
We have formulated and implemented physical health intervention plans for special groups such as adolescents, women, the elderly, occupational groups and the disabled. We will carry out a plan to promote youth sports activities, cultivate young people's hobbies in sports, basically realize that young people can master at least one sport skill, and ensure that students have at least one hour of physical activity in school every day. By 2030, 100 percent of school sports facilities and equipment will be up to standard, young students will participate in moderate intensity sports activities at least three times a week, and more than 25 percent of students will meet the national physical health standards. Chapter 7 Strengthening public health services covering the whole population
Section 1 Prevention and Treatment of serious diseases
We will implement the strategy for the comprehensive prevention and control of chronic diseases and strengthen the development of national demonstration zones for the comprehensive prevention and control of chronic diseases. We will strengthen screening and early detection of chronic diseases, carry out early diagnosis and treatment of key cancers in high-incidence areas, and promote opportunistic screening of chronic diseases such as cancer, stroke and coronary heart disease. We will basically achieve full coverage of management and intervention for patients with hypertension and diabetes, and gradually incorporate appropriate technologies for the early diagnosis and treatment of cancer, stroke and other major chronic diseases into the treatment routine. We will strengthen prevention and treatment of common diseases such as myopia and obesity among students. By 2030, the health management of chronic diseases should be implemented across the population and the whole life cycle, and the overall 5-year survival rate of cancer should be increased by 15 percent. Strengthen oral hygiene, and control the caries rate of children aged 12 within 25%.
We will strengthen prevention and control of major infectious diseases. We will improve the monitoring and early warning mechanism for infectious diseases. We continued to expand the national immunization program, kept the vaccination rate of school-age children under the program at a relatively high level, and established an insurance mechanism to compensate for abnormal reactions to vaccination. Management of HIV testing, antiviral treatment and follow-up has been strengthened, and clinical blood nucleic acid testing and prevention of mother-to-child transmission of HIV have been fully implemented to keep the epidemic at a low epidemic level. China has established a comprehensive service model for tuberculosis prevention and control, strengthened the screening and monitoring of multidrug-resistant tuberculosis, and standardized the management of tuberculosis diagnosis and treatment. We will effectively respond to epidemics of influenza, hand, foot and mouth disease, dengue fever, measles and other major infectious diseases. The comprehensive strategy of schistosomiasis control focusing on controlling infectious sources has been continued, and all endemic counties have reached the standard of schistosomiasis elimination. Continue to consolidate the country's progress in eliminating malaria. Hydatid disease and other major parasitic diseases were basically brought under control in all endemic counties. We will continue to control and eliminate major endemic diseases so that they will no longer be a major problem endangering people's health. We will strengthen prevention and control of acute outbreaks of infectious diseases, actively guard against imported acute outbreaks of infectious diseases, and strengthen prevention and control of plague and other traditional severe infectious diseases. We will strengthen control of major animal-borne infectious diseases at the source.
Section 2 Improving the administration of family planning Services
We will improve the integrated decision-making mechanism for population and development and the policy system conducive to balanced population development. We will reform the way we manage family planning services, pay more attention to serving families, and establish a policy framework for family development that focuses on childbearing support, child rearing, youth development, elderly support, and care for the sick and disabled, so as to guide the people to have children in a responsible and planned way. We will improve the national policy on technical services for family planning, and strengthen the guarantee of technical services for second family planning. We will comprehensively promote informed choice and popularize the knowledge of contraception and reproductive health. We will improve the system of reward and assistance for family planning families and the system of special assistance, and dynamically adjust the standard of reward and assistance. We will adhere to and improve the family planning target management responsibility system, and improve the long-term working mechanism of family planning featuring publicity and advocacy, law-based management, quality services, policy promotion and comprehensive management. We will establish a sound mechanism for monitoring births. We will continue to control the sex ratio at birth. By 2030, China will achieve a natural balance in the sex ratio at birth.
Section 3 Promoting Equal access to basic public health services
We will continue to improve the national basic public health service projects and major public health service projects, strengthen research on the economic burden of diseases, adjust the funding standards for projects when appropriate, constantly enrich and expand the content of services, and improve the quality of services, so that urban and rural residents can enjoy equal access to basic public health services, and ensure equal access to basic public health and family planning services for the floating population.
Chapter 8 Provision of Quality and Efficient Medical Services
Section 1 Improving the medical and health service system
An integrated medical and health service system featuring a complete system, clear division of labor, complementary functions, close collaboration and efficient operation has been fully established. Basic medical and health resources within counties and cities should be rationally distributed according to the resident population and service radius to ensure equal access to basic medical and health services for all. At the provincial level and above, regional medical resources should be allocated in a coordinated manner, and regional medical resources should be shared in an integrated way, so as to basically achieve balanced allocation of high-quality medical and health resources, and ensure that everyone in the province enjoys homogeneous diagnosis and treatment of critical and difficult diseases and specialized medical services. Relying on existing institutions, China will build a number of national medical centers with leading domestic and global influence, a number of regional medical centers and national key clinical specialty groups, promote the coordinated development of medical and health services in the Beijing-Tianjin-Hebei region and the Yangtze River Economic Belt, and drive regional development and overall improvement of medical services. We will strengthen the development of complementary medical institutions for rehabilitation, geriatric diseases, long-term care, chronic disease management, and hospice care. We will implement the health poverty alleviation project, increase support for the construction of medical and health institutions in poor areas in the central and western regions, improve their service capacity, and ensure the health of the poor population. By 2030, there will be 4.7 registered nurses for every 1,000 permanent residents.
Section 2 Innovating the supply mode of medical and health services
We have established a three-in-one mechanism for the prevention and control of major diseases, featuring specialized public health institutions, general and specialized hospitals, and community-level medical and health institutions. We have established an information sharing and connectivity mechanism, and promoted the integrated development of chronic disease prevention, treatment, and management, so as to realize the integration of medical treatment and prevention. We have established a division of labor and cooperation mechanism with clear goals, powers and responsibilities among medical and health institutions of different levels, categories and organizations, and constantly improved the service network, operation mechanism and incentive mechanism, so that the community is generally equipped with the ability to be the gatekeeper of residents' health.
